The schedule for this position is as follows:
Saturday: 8 AM - 8 PM
Sunday: 8 AM - 8 PM
FUNCTIONAL/TECHNICAL REQUIREMENTS:
- Implements services, both in the individual’s home and in the community that is based on the Person-Centered Individual Support Plan (PCISP). These services may include personal assistance such as bathing, transferring, and toileting the person served. They may also include administering medication, assisting with tube feeding, and meal preparation for the individual.
- Implements High-Risk Plans as well as Behavior Support Plans as applicable.
- Completes housekeeping duties to create a safe and healthy environment for individuals served. Engages persons served in cleaning and cooking duties as outlined in Person Centered Individual Support Plan (PCISP).
- Transports persons served, as needed, to community activities and appointments.
- Completes all documentation on a timely basis, as related to the provision of services. Assures that documentation is timely, thorough, and accurate, meeting state and Medicaid Waiver requirements.
- Maintains an awareness of and ability to quickly access information relating to an individual.
- Monitors the safety of individuals served and immediately reports and documents injuries, accidents, and incidents following appropriate procedures.
- Immediately reports suspected abuse (verbal, physical, sexual) neglect, mistreatment, and/or exploitation of an individual served to the Manager.
- Adheres to HIPAA regulations and Evansville ARC policy regarding the privacy and confidentiality of individuals served and their families.
MINIMUM QUALIFICATIONS:
- Proficient in computer programs routinely used.
- High school diploma or general education degree (GED).
- Must be at least 18 years of age.
- Must pass a Department of Motor Vehicles background check and hold a valid driver’s license.
- Must have and maintain an acceptable driving record, i.e., less than 3 moving violations within 3 years.
- Maintains certification for First Aid and CPR and Core A and B for medication administration.
- Ability to work daily with clients who exhibit potentially aggressive behavior.
PHYSICAL DEMANDS:
- Requires the ability to hear, see, talk, read and write on a constant basis.
- Regularly required to stand; use hands to finger, handle or feel; reach with hands/arms and talk or hear.
- Frequent keyboarding on the computer.
- Ability to regularly lift and move 50 pounds
- Must be able to work in a variety of community settings which may require sitting, standing, and stooping for extended periods.
WORKING CONDITIONS/ENVIRONMENT:
- Must have access to a reliable and safe vehicle for transportation of individuals served. Employees must be willing and able to drive their personnel vehicle for the provision of services. Mileage reimbursement is provided.
- Must be able to work nonstandard/flexible hours, including some weekend, evening, and holiday hours to meet individual’s needs.
- May be exposed to pets in the clients’ home such as dogs or cats.
